“Swap failed” in a browser is not a precise blockchain state. Before changing fees or signing again, establish whether a transaction exists and whether the chain executed it.

The receipt status distinction follows EIP-658. A missing receipt alone cannot distinguish every pre-inclusion state.
Use the correct identity
Check the full transaction hash on the chain where you signed. The same account address can have unrelated activity on several chains. Confirm the sender and transaction nonce, particularly if the wallet shows a speed-up or cancellation.
A pending transaction needs a waiting or replacement decision. A mined failure needs diagnosis and, if appropriate, a newly reviewed transaction. Treating those as the same problem can create an unnecessary second trade.
When the explorer cannot find it
Check account history and the wallet’s submission details. Private routing can limit public visibility before inclusion. A dropped transaction can also reappear if rebroadcast, as Etherscan explains. “Not found” therefore does not prove permanent cancellation.
Keep a short record of the chain, sender, nonce and every related hash. That record makes it possible to identify which request eventually executes without relying on changing interface labels.
